Students with special health concerns(asthma, seizure, disorder, diabetes, severe food allergies, etc.) must have an emergency action plan on file every school year. This information is curcial to keep your child safe during the school day. Please print the appropriate form and take it to your child's doctor. Return the form to school promptly with the required emergency medication and medication forms.
